Dr. Sury Jairaj is the consulting physician for The Herbal Realm’s Medical Marijuana Certification Program. He provides compassionate, professional evaluations for patients seeking initial certification or annual renewal under Pennsylvania’s Medical Marijuana Program.
During your appointment, Dr. Jairaj will review your medical history and determine whether you qualify for medical cannabis under Pennsylvania Department of Health guidelines. If approved, he will submit your physician certification to the state so you can complete your registration and receive your medical marijuana card.

3. Pay the State Fee
After you’ve been certified, log into your patient profile and submit the $50 annual fee. (Discounts may apply for Medicaid, SNAP, or PACE participants.)
4. Receive Your Card by Mail
Your card will be mailed to you within 7–10 business days. Once received, you’ll be able to shop at any licensed dispensary in Pennsylvania.
